Role Overview:

Responsibilities and Duties:
Conduct assessments to determine the needs, strengths, and goals of individuals with disabilities.Develop and implement individualised support plans in collaboration with clients and stakeholders. Coordinate service delivery and liaise with community organisations and healthcare providers. Provide information, guidance, and advocacy to individuals with disabilities and their families. Facilitate skill-building workshops and social events to enhance quality of life. Monitor and review support plans, making adjustments as necessary. Maintain accurate records and documentation in compliance with regulations. Collaborate with multidisciplinary teams to address holistic needs. Stay updated on developments in disability services through professional development.
Key Selection Criteria:
Experience in the disability sector, preferably in a similar support and advocacy role.Knowledge of disability legislation, policies, and service delivery frameworks. Strong communication and interpersonal skills. Ability to assess needs and develop person-centered support plans. Capacity to work independently and manage time effectively. Commitment to upholding the rights and dignity of individuals with disabilities. Relevant qualifications in disability services or a related field.
